Children’s passports
Please bring the child to the Consular Section with you when you apply for the passport. Passport Issuance for Children (under 16)
- Complete form DS-11 (do not sign)
- Submit current US passport
- Original birth certificate/Consular Report of Birth Abroad/adoption decree with parents’ and child’s name shown on it
- Two 2x2” (5x5 cm) passport photos – see photo requirements
- $85 USD (or equivalent RMB), accepted in cash (USD or RMB)
Passport renewal for Children (under 16)
- Your child’s previous passport
- Your child’s birth certificate/Consular report of birth abroad/adoption decree, the document should be either the original or a certified copy
- Complete form DS-11 (do not sign)
- Two 2x2” (5x5 cm) passport photos – see photo requirements
- The child’s Social Security Number is required on the application form
- Parents’ or guardians’ passports
- $85 USD (or equivalent RMB), accepted in cash (USD or RMB)
For children under 16 both parents need to be present and provide unequivocal consent to passport issuance by signing the child’s passport application form in front of the Consular officer.
If one parent is unable to be present for the application, the parent must provide a signed and notarized statement of consent (form DS-3053) or a notarized letter of consent, in English, authorizing the other parent to have the child’s passport issued.
Alternatively, the parent applying for the passport must submit proof of sole custody.
If the child was a baby when the last passport was issued, please bring photos that show the child’s development over time.

- Submit current passport
- Complete form DS-4085
- One parent needs to appear in person and bring the child's birth certificate/Consular report of birth abroad/adoption decree which should be original or certified copy. Child doesn’t have to come in person.
