Adult passports

First time passport issuance for adults (aged 16 or older)

1. Proof of citizenship (e.g. sealed birth certificate, Consular Report of Birth Abroad (form FS-240), Certificate of Naturalization)
2. One photo identification card
3. Two recent 2x2’ (5x5cm) photos – see photo requirements
4. Complete for DS-11 (do not sign)
   
5. $100 USD (or equivalent RMB) payment, accepted in cash (RMB or USD)

 
Passport renewal for adults (aged 16 or older)

1. Submit current or expired passport
2. Two recent 2x2’ (5x5 cm) photos – see photo requirements
3. If you present passport was valid for 10 years, complete form DS-82. If your present passport was only valid for 5 years, complete form DS-11 (do not sign)
4. $75 USD (or equivalent RMB), accepted in cash (USD or RMB)

1.      Submit current passport

2.      Complete form DS-4085

Note: The consular officer has the right to request additional information. Please do not sign application forms until directed/asked to do so.
